Surgical Management of Sacral Osteomyelitis & Discitis After Sacrocolpopexy

Objectives: To review the clinical presentation, diagnostic methods and management strategies of osteomyelitis and discitis.

Methods: We present two clinical cases demonstrating early and later onset presentations of sacral osteomyelitis following minimally invasive sacrocolpopexy with concurrent hysterectomy, necessitating surgical intervention.

Clinical Relevance: Sacral osteomyelitis, a rare yet serious complication following sacrocolpopexy, is often linked to direct inoculation from traumatic or deeper placement of sacral sutures. Timely recognition of clinical signs, particularly back pain or radiculopathy, is imperative. MRI serves as the preferred imaging modality although confirmatory findings on MRI may lag behind clinical symptoms. Therefore, management should be based on clinical assessment rather than imaging findings alone. Treatment typically entails prolonged antibiotic therapy and removal of sacral sutures. Neurosurgical intervention for persistent pain despite suture removal should be considered.
